Health Partners Release Community Health Survey

Black Hawk County Public Health, MercyOne, Peoples Community Health Clinic, and UnityPoint Health are releasing a community health survey to obtain the public’s input on health needs, gaps, and existing community strengths.

The survey is part of a larger assessment which will result in a comprehensive report of health assets, needs, and disparities. A summary of the data will be released in early 2025 for the public to help identify health priorities. The 19 organizations that make up the Black Hawk County Community Health Improvement Steering Committee will then mobilize working groups to develop and monitor improvement plans. The plan will serve as the roadmap for the Black Hawk County community to collaborate on health improvement strategies for the identified priority areas.

All adults that live, work, or visit Black Hawk County are encouraged to fill out the confidential, anonymous survey by clicking the following link.

The survey is open through the end of September and available in multiple languages. Paper copies of the survey may be requested by calling Black Hawk County Public Health at (319) 291-2413.

“The Community Health Assessment is a great example of a process where communities work together to create a place where all people want to live, feel welcome, and have opportunities to thrive,” said Kaitlin Emrich, Black Hawk County Public Health Director. She added, “Everyone benefits from the collaboration between public health and healthcare because we’re all invested in understanding the health needs of the community and moving collectively to address the priority issues identified from the assessment.”
